As a caregiver of a family member, friend or loved one living with FTD, you are often called upon to be the voice of the person you are caring for, to learn how to be an advocate and to make decisions on their behalf. Patient advocacy is about communicating values and goals of care for someone who may not be able to speak for themselves. Community advocacy activities can empower the authentic voice of caregivers and persons living with a diagnosis, promoting person-centered care planning and dignity at every stage of care. This session will provide a framework for creating a care plan that will help you connect with your community for support, develop your skills as an advocate, communicate with healthcare providers and ensure that your values are incorporated at every stage of care.
